This cultivar was listed on the Hosta Lists website.  Originated by Jeff and Marie White of Iowa and Rod Kuenster of Iowa, this non-registered cultivar is a seedling of a seedling of H. 'Crazy Quilt' × H. 'Beer Belly Blues'. It is a medium size hosta with blue-green foliage that has creamy white streaked variegation.
